At Mercy Housing Lakefront, home is just the beginning. We partner with community organizations to offer programs and services to help meet residents’ needs. At Johnston Center Residences in Milwaukee, eight residents participated in a four-week Medical Homes Program, sponsored by healthcare partner City on a Hill (COAH).
Participants learned healthy lifestyle habits, preventative care education to improve their health, and how to effectively use the healthcare system to find a medical home with a primary care physician. At the end of the program they received a certificate of completion and a basket of household goods.
The partnership with COAH also includes a monthly visit from LaTrice Luckett, a Community Health Worker, who provides residents with monthly health and wellness education, biometric screenings, and healthcare navigation.
